If you are selling product at your booth space, you MUST complete a Leadville Business License Form. Click to download: 2019 Business License Application. Life Time will pay the fee on our vendors’ behalf. If a Business License is not complete in advance of the event, you will not be permitted to sell.
If you are selling product at your booth space, you are responsible to pay State and Lake County tax:
- If you already have a CO state sales tax number, please fill out the following document to complete your Lake County Sales Tax Application and include your state sales tax number, note as “Sales Tax Special Events Sales Tax Return.” Click to download: County Sales Tax Application.
- If you do not have a CO state sales tax number, please fill out an online application via the Department of Revenue or in-person in Denver.

SAMPLING FOOD & BEVERAGE
If you are sampling food and/or beverage, you are required to complete THIS DOCUMENT, which is a Lake County Temporary Food Event Vendor Application. A fee is not required if you are sampling only.
SELLING FOOD & BEVERAGE
If you are selling food and/or beverage, you are required to complete THIS DOCUMENT, which is the same as above – a Lake County Temporary Food Event Vendor Application. Note that there is a fee for selling food and/or beverage, which can be paid directly to the Lake County Health Department.
If you are selling or sampling food and/or beverages and are already licensed in the state of Colorado, please send me a copy of your Colorado Retail Food Establishment License and complete page one in the Lake County Temporary Food Event Vendor Application.
